The City of North Myrtle Beach is committed to improving the health and wellbeing of our employees. As an employee, you have access to the Beach Healthy Program. The goal of the program is to create a healthier workplace.

Program Benefits:

➢Biometric Screening completed once a program year. The program year is October 1, 2022 – November 1, 2023

➢ Access to City of North Myrtle Beach Clinic

➢ Earn a health insurance premium reduction and up to $505 deposit in your Health Reimbursement Account

Eligibility

Employees and spouses enrolled in the health plan are eligible for a premium reduction and up to $505 deposit into their Health Reimbursement Account in January 2024.

 Employees and spouses hired between 10/1/2022 and 9/30/2023 are grandfathered in for the premium reduction and have until 11/1/2023 to complete all program expectations to be eligible for premium reduction and up to $505 deposit in your Health Reimbursement Account for 2024.

Retirees and spouses on the City’s health plan, are eligible for up to $505 deposit into their Health Reimbursement Account in January 2024.

How do I get started:

 Register or Login

  • Existing users in the inHealth wellbeing app click “Login”
  • New users download the inHealth wellbeing app to register. If you don’t have access to a smartphone, you can conveniently login on a PC at https://portal. inhealth4change.com
  • Click on “Here” to create an account
  • Enter company ID: CNMB
  • Press “create” to submit information

Step 1: Complete the “My Health Risk Assessment Questionnaire” before your biometric screening.

  •  Visit the mobile app or web portal at https://portal.inhealth4change.com/ and locate the screening tab to the left of your screen.
  • Once you’ve clicked on the screening tab, you’ll see the option to complete your Health Risk Assessment Questionnaire.

Step 2: Schedule and complete your biometric screening in January 2023

  • After completing and submitting your Health Risk Assessment Questionnaire, contact the Employee Health Center to schedule your biometric screening.
  • Have your employee ID number available.
  • Fast 6 – 8 hours before screening. You may take any medications with water while fasting.
  • If an employee chooses to go to their PCP, a signed, dated and completed Biometric Screening Results must be submitted to the employee Health Clinic no later than February 28th

Step 3: Complete annual wellness consultation ·                   

  • Schedule an annual wellness consultation before or during your departmental month at the Employee Health Center. Contact the clinic at 843-366-2714 to schedule your consultation.
  • If an employee chooses to go to their PCP, a signed, dated and completed Provider Consultation Form must be submitted to the employee Health Clinic no later than the last day of your scheduled department month. Click here for form.

A biometric screening is a series of health tests designed to offer insight into your overall health. 

Q:  How do I schedule a biometric screening?

A: Attend a screening event or download the Biometric Screening Form located on the portal/app and have your PCP complete the entire form and sign it. Return it to CNMB Employee Health Center by February 28, 2023.

Q:  Who is eligible to complete a biometric screening?

A:  Employees and retirees and their spouses enrolled on the health insurance plan (medical coverage) are eligible to complete a biometric screening.

Q:  Why should I complete a biometric screening?

  • Continue to take the first steps on your path to living a healthier, longer life. The biometric screening may help you identify a health-related problem you didn’t know you had.
  • To qualify for premium reductions and/or Health Reimbursement Account deposit of up to $505.

Q: What must I do before my biometric screening?

A: Login to your inHealth Wellbeing account through the inHealth Wellbeing mobile app or https://portal.inhealth4change.com

Click on the screening tab located to the left of your screen.

Complete your Health Risk Assessment Questionnaire

Fasting at least 6 hours (including avoiding gum, mints, coffee creamer, etc.) before your screening is recommended for best results.  YOU MAY TAKE ANY MEDICATIONS WITH WATER WHILE FASTING.  

After your biometric screening has been completed and submitted, you will receive an email with a link to check your results and view your rewards! 

Q:  What if I previously had my blood work done with my Primary Care Provider?

A:  If you had your biometrics completed after December 1st, you can submit the results to CNMB Employee Health Center via the Biometric Screening Form.The form must be completed in its entirety.

Q:  What measurements are taken during a biometric screening?

A:  Refer to the Biometric Screening Form for details on biometrics measured.

Q:  How do I view my biometric screening results?

A:  Login to your inHealth Wellbeing account. Click on Screening in the left menu. You will be able to view your current results side-by-side with your first screening results in the Member Results Dashboard. You may also review in-depth summaries by clicking “Download My Results”, click on the Health Risk tab, and click on the My Results tab to view.

Q:  How often do I need to complete the Health Risk Questionnaire and perform a biometric screening?

A:  Once per program year for both. 

Q:  Can I complete my biometric screening with my primary care provider (PCP) through an annual wellness visit?

A: Yes, download the Biometric Screening Form located on the portal/app and have your PCP complete the entire form and sign it. Return it to the CNMB Employee Health Center by February 28, 2023.

Q: Can I complete my results consultation with my PCP?

A: If you already have a primary care physician that you would prefer to discuss your results with, you can request your lab results by emailing the Employee Health Center at healthcenter@nmb.us (Be sure you indicate how you want to receive your results—mail, pick up at EHC, sent through interoffice mail). Please allow the EHC up to 2 business days to process your request. Review your results with your physician and have him/her complete the Provider Consultation Form. The completed form must be submitted to the Employee Health Center no later than the end of your department’s scheduled month. If you review your results with your physician, please note:

• The physician must complete the Provider Consultation Form in its entirety

• You will be responsible for any copays and/or lab fees associated with your consult visit.  You may still be required to meet with the clinic to review any incomplete sections on the form to include nicotine testing results.

Q:  If I improve my biometric screening results during the program year how do I submit my new values?

A:  Participants may submit a new Biometrics Provider Screening Form indicating the new values to McLeod Onsite Health. Your primary care provider must complete the entire form and sign the form. Or, you may schedule a follow up visit at the CNMB Employee Health Center for recheck.

Biometric Screening Form

Participation in the Wellness Incentive Plan is optional; however, there are numerous benefits to participating:

  • Save money on insurance premiums and employees and spouses on the health plan can each earn up to $505 in HRA deposits
  • Employee Only Coverage—you save $1092 per year in premiums
  • Family Coverage—you save $2808 per year in premiums
  • Employees (and spouses on the City’s insurance) can also earn up to $505 in HRA funds each year. These funds do not expire while you are an active full-time employee and can be used towards any medical expenses.

Q:  Can you tell me about the wellness rewards?

A:  You can earn up to $505 each program year by working toward a healthier lifestyle. You have from January 1, 2023, until November 1, 2023 to earn a premium reduction and HRA deposit.  All the details are below:

  • You must currently be enrolled in the City of North Myrtle Beach insurance plan to redeem your rewards.
  • Have created an account in the inHealth app/portal
  • Complete your Health Risk Questionnaire.
  • Attended a Biometric Screening or return the Biometric Screening Form from your PCP by 2/28/23.
  • Get an annual wellness visit at the CNMB clinic (during your department month) or with your Primary Care Provider by 9/30/2023.
  • Achieve healthy ranges for Body Mass Index (BMI), blood pressure, cholesterol, and blood glucose If you do not achieve one or more of these health goals during a biometric screening, you may complete one of our wellness programs or challenges to earn credit.
  • Test negative for nicotine or complete a Tobacco Cessation Program by 11/15/23.

 

 

Q:  What if I did not earn the wellness rewards for Body Mass Index (BMI), blood pressure, cholesterol and/or blood glucose during my biometric screening with the McLeod OnSite Health team or with a primary care provider?

A:  Eligible participants have until November 1, 2023, to earn these wellness rewards by completing a city sponsored wellness challenge, participating in 1:1 health coaching or by submitting a RAS form or submitting updated values to the CNMB Health Center.  Participants can earn the Tobacco/Nicotine Free incentive by testing negative or completing an approved tobacco cessation program by 11/1/2023.

A. Submit a help ticket via the app or the portal at https://portal.inhealth4change.com.

Q. What do I do if I forgot my password to the inHealth mobile app?
A. Click “Forgot Password” on the sign in screen. A temporary password will be emailed to you.
