Your Company ID can be found in the program message you received at the start of the program year. Can’t find it? Contact your wellbeing program administrator.
Enter your email address and create a secure password.
• Must be 8–15 characters
• Include at least one number and one special character (!@#$%^&*)
Provide your first and last name, date of birth, last 6 digits of your Social Security Number, race, sex, and physician name. Click Next to continue.
Read the terms and conditions and privacy policy. Check the box to confirm you agree.
Your account has been created. Click Back to login to return to the login screen.
Sign in with your email and the password you just created.
