Florence County is committed to improving the health and wellbeing of our employees. As an employee, you have access to the Florence County Commit to Be Fit Program. The program is designed to provide you with the resources and information you need to Start Living the Life You Want.

Florence County Commit to be fit program Year 2 will begin in February 2023 and ends October 31, 2023

Eligibility

All employees are eligible to earn up to $200 in gift cards.

  • $100 gift card by participating in biometric screening
  • $100 gift card in incentives.

The program is available at no cost to employees who have PEBA insurance.

Employees who are NOT covered by PEBA insurance are eligible. There is a cost of $24.75 for lab work required to complete a biometric screening. The charge will be payroll deducted.

Additional lab tests are available to all employees for an extra fee that will be payroll deducted. See “Additional Testing” tab

To get started earning rewards:

Step 1:

Register or Login

  • Existing users in the inHealth wellbeing app click “Login”
  • New users download the inHealth wellbeing app to register. If you don’t have access to a smartphone, you can conveniently login on a PC at https://portal. inhealth4change.com
  • Click on “Here” to create an account
  • Enter company ID: FLOCO
  • Press “create” to submit information

Step 2: Complete Health Risk Assessment (HRA) Questionnaire & Schedule Your Appointment

  • Visit the mobile app or web portal at https://portal.inhealth4change.com/ and locate the screening tab to the left of your screen.
  • Once you’ve clicked on the screening tab, you’ll see the option to complete your Health Risk Assessment Questionnaire.
  • After completing and submitting your Health Risk Assessment Questionnaire, attend a screening event in February or contact Charlene Conner at 843.292.7432 to schedule your biometric screening.

Step 3: Complete your biometric screening in February 2023 or within 60 days of hire date.

Step 4: Complete a wellness visit to either the Nurse Practitioner or Primary Care Physician to review the screening results before October 31, 2023.

  • Contact the Employee Health Center to schedule 843-676-1215 a follow up visit with the Nurse Practitioner.
  • If completing the wellness visit with your PCP, the completed form must be returned to the Employee Health Center by October 31, 2023. Contact Charlene Conner for the PCP wellness form at 843-292-7432.

A biometric screening is a series of health tests designed to offer insight into your overall health.  A biometric screening includes measurements of height, weight, waist circumference, Body Mass Index (BMI), blood pressure, heart rate, A1C, total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides.

Q: Why should I screen?

A: You will receive a free wellness screening, a comprehensive health risk assessment, and a one-on-one health coaching session to help you take the first steps on your path to living a healthier, longer life. The biometric screening may even help you identify a problem you didn’t know you had. Employees who screen and have a Wellness Exam with the Nurse Practitioner at the Employee Health Center or with their Primary Care Physician to review their results will qualify to earn a $100 gift card.

Q: What are the steps for my biometric screening?

A:

Step 1: Login to your inHealth Wellbeing account through the inHealth Wellbeing mobile app or https://portal.inhealth4change.com.  Once you’ve clicked on the screening tab, you’ll see the option to complete your Health Risk Assessment Questionnaire. This must be completed PRIOR to attending the biometric screening.

Step 2: Attend a screening event in February 2023

February 1 -Complex – 8am – 12pm

February 8 -Complex – 8am – 12pm

February 15 -LEC – 9am – 12pm

February 22 – Library – 9am – 12pm

Fasting at least 6 hours (including avoiding gum, mints, coffee creamer, etc.) before your screening is recommended for best results.  YOU MAY TAKE ANY MEDICATIONS WITH WATER WHILE FASTING.

Q:  What measurements must be taken during a biometric screening?

A:  Height, weight, waist circumference, Body Mass Index (BMI), blood pressure, heart rate, blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides

Q:  How do I view my biometric screening results?

A:  Login to your inHealth Wellbeing account. Click on Screening Results in the left menu. You will be able to view your current results side-by-side with your first screening results in the Member Results Dashboard. You may also review in-depth summaries by clicking “Download My Results” click on the Health Risk tab and click on the My Results tab to view.

Q:  How often do I need to complete the Health Risk Questionnaire and perform a biometric screening?

A:  Once per program year for both. 

Q:  Can I complete my biometric screening with my primary care provider (PCP) through an annual wellness visit?

A: No. All biometric screenings must be completed at a Screening Event in February 2023. New hires will be screened at the Employee Health Center within 60 days of date of hire.

Q:  Can you tell me about the wellness rewards?

To earn rewards, you must have created an account in the inHealth app/portal

A:  You will receive $100 gift card for completing:

  1. Health risk assessment
  2. Biometric screening
  3. Wellness Exam to review biometric screening results with the Nurse Practitioner at the employee health clinic, or with your Primary Care Physician.

Contact the employee health clinic at 843-676-1215 to schedule a Wellness Exam with the NP. The PCP form is available at florenceco.org Intranet to be completed by your PCP.

Participants will be able to earn up to an additional $100 gift card by demonstrating a healthy range in below measures or completing 1:1 care management at the employee clinic.

Q:  When will I receive my gift cards?

A: Gifts cards will be distributed by HR in December 2023, the end of the program year for all eligible participants.

You must currently be employed by Florence County and meet program requirements to receive your gift cards.

Q:  What if I did not earn the wellness rewards for Body Mass Index (BMI), blood pressure, cholesterol and/or blood glucose during my biometric screening or with a primary care provider?

A:  Eligible participants have until October 31, 2023, to earn these wellness rewards by completing our complimentary 1:1 care management, or by submitting updated values from your PCP.

Additional Tests and Cost:

Prostate Specific Antigen (PSA): $10.50

  • US Preventive Service Task Force Grade C for Men 55 – 69 years. (Offer or provide this service for selected patients depending on individual circumstances).
  • This test looks for PSA, a substance that may be found in high amounts in men with prostate cancer. However, a high PSA level does not always mean that a man has prostate cancer. PSA levels may be high in men with other types of prostate problems.
  • Screening offers a small potential benefit of reducing the chance of death from prostate cancer in some men. However, many men will experience potential harms of screening, including false-positive results that require additional testing and possible prostate biopsy; over diagnosis and overtreatment; and treatment complications, such as incontinence and erectile dysfunction.

Blood Type: ABO/Rh Typing $13.50

  • Determination of ABO blood group and Rh type.
  • Group and typing of expectant mothers and newborns may indicate potential for ABO hemolytic disease of the fetus/newborn. Rh typing is used to determine Rh immune globulin candidacy for prenatal and postpartum patients.

Vitamin B12 AND Folate: $21.50

  • Vitamin B12 is required for proper red blood cell formation, neurological function, and DNA synthesis.
  • Those at risk for deficiency of this vitamin include older adults, individuals with pernicious anemia, individuals with gastrointestinal disorders/surgery, or those not getting it in dietary intake due to limiting animal-based foods (vegans, strict vegetarians, some pregnant or lactating women).

C-Reactive Protein (CRP): $9.00

  • CRP can be used as a test for inflammatory diseases, infections, and neoplastic diseases. Progressive increases correlate with increases of inflammation/injury.

Testosterone Serum: $18.00

  • Testosterone testing is used to evaluate androgen excess or deficiency related to gonadal function, adrenal function, or tumor activity. Testosterone levels may be helpful in men for the diagnosis of hypogonadism, hypopituitarism, Klinefelter syndrome, and impotence (low values). Testosterone levels may be requested in women to investigate the cause of hirsutism, anovulation, amenorrhea, virilization, masculinizing tumors of ovary, tumors of the adrenal cortices, and congenital adrenal hyperplasia (high values).
  • Measurements should be obtained between 8 – 10 am. Most accurate testing occurs in the fasting state.
