Through a comprehensive and inclusive process, inHealth can gather both quantitative and qualitative data to create a roadmap for a community that turns analytics into actionable steps. This process both broadens the coalition of partners working to change health outcomes in the community and broadens the definition of health to include social determinants of health and environmental factors. In many communities, the resources required to change health outcomes are already being expended, but in an uncoordinated or haphazard fashion that doesn’t achieve the desired results.
A successful implementation of a meaningful health system or local Community Health Needs Assessment and Implementation Strategy (CHNA-IS) process includes, and can have pieces led by, health systems, government, foundations, community organizations, educational institutions, and the business community. Engagement with inHealth will not only ensure compatibility with all Affordable Care Act regulations, but transform the purpose and potential of the CHNA-IS process.
