Q: What is Healthy Lives?

Working well begins with living well.  As a benefit to every team member, the Franciscan Missionaries of Our Lady Health System (FMOLHS) offers the Healthy Lives Wellness Program.  Healthy Lives program design provides support and encouragement to individuals to achieve their health goals.  All team members can participate at no cost. At home or work, these tools, along with personal coaching, help each team member and their family identify what is important to their health and well-being with a plan to achieve results.  Body, mind, and spirit – the Healthy Lives Wellness approach is comprehensive and complements your personal physician’s care and personal health goals.

Q: How do I become a member of Healthy Lives?

To become part of Healthy Lives, you must first be an employee of the Franciscan Missionaries of Our Lady Health System.  If you meet the eligibility requirements, follow steps 1 – 4 below to have the opportunity to earn up to $550 in wellness rewards in the program year of November 1, 2020 – October 31, 2021.

Step 1: Have an active account in the inHealth Wellbeing portal or mobile app. Click here for how to create an account.

Step 2: Complete the ‘My Health Risk Questionnaire’ located on the Health Risk tab.

Step 3: Schedule and complete a wellness screening before October 31, 2021.

Q: Who is eligible to participate in Healthy Lives?

All FMOLHS team members are eligible to participate in the Healthy Lives Wellness Program.

Q: What if I am unable to participate due to a limitation or disability?

Incentives may be available for team members who participate in certain health-related activities or achieve specific health outcomes. Suppose you cannot participate in any wellness program health-related activities or achieve any of the health outcomes required to earn an incentive. In that case, you may be entitled to reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting Healthy Lives at 855-426-4325.

Q: If I’m a new employee of the health system, how long do I have to complete a biometric screening?

All team members must screen by October 31st to qualify for a cash incentive.  Contact Healthy Lives at 1-855-426-4325 to schedule a private screening appointment with a health coach.

Biometric Screening

A biometric screening establishes a health baseline used to evaluate the risk for various health issues, many of which are prevented through early detection and lifestyle changes.  At Healthy Lives, we use the information from biometric screening to help our members stay healthy.

Q: Who is eligible to complete a biometric screening?

All FMOLHS team members are eligible to complete a Healthy Lives screening at no cost.

 Q: Why should I complete a biometric screening?

Completing a biometric screening team members can:

  • Take the first steps on the path to living a healthier lifestyle. A biometric screening may help you identify a health-related problem you didn’t know you had.
  • Earn a cash incentive of up to $550.

Q: Why do I need to complete a Health Risk Questionnaire?

Preventable chronic conditions contribute to 75% of health care costs. Heart disease, for example, can be prevented with proper diet and exercise. Early identification of certain risk factors, such as high cholesterol or blood pressure, can prevent more significant problems down the road.

This is where biometric screenings are most beneficial.  A regular assessment of an individual’s health can provide valuable insight into current or potential medical issues and their risk for chronic, preventable diseases such as diabetes, heart disease, and hypertension. It can identify strategies for improved health and well-being.  

Q: How do I schedule a biometric screening?

All FMOLHS team members may participate in a biometric screening.  Screening appointments are available from February 1 – October 31, 2021.  Follow these steps for completing your biometric screening:

Step 1: Create your account and complete your Healthy Lives Screening Questionnaire.

If you already have an account on the inHealth mobile app or web-based portal, enter your email address and password, and locate the Screening tab to the left of your screen.

If this is your first time using the inHealth mobile app or web-based portal, download the inHealth Well-being app from the App Store or Google Play. Search ‘inHealth: Well-being.’

Click ‘Here’ to create an account under First Time Here? and follow these steps to create your account and complete your questionnaire.

  • Enter Company ID: FMOLHS and click ‘Next.’
  • Complete the demographic information and create a password. The password must be between eight (8) and 15 characters and must contain at least one number and one special character (accepted special character are ! @ # $ % ^ & *).
  • Review the Terms of Service and Privacy Policy and mark the ‘I Agree’ checkbox.
  • Click ‘Create.’ After creating an account, you will return to the Login/Create Account
  • Enter your email address and password and click ‘Login.’

Step 2: Complete Health Risk Questionnaire and Schedule Your Screening.

  • Once you’ve clicked on the Screening tab, you’ll see the option to complete your Health Risk Assessment Questionnaire.
  • After completing and submitting your Health Risk Assessment Questionnaire, you will be directed to complete or schedule your biometric screening.
  • Click “Schedule Biometric Screening” for instructions on how to schedule your screening.

Step 3: Attend a screening from February 1 – October 31, 2021.

  • For best results, recommended fasting 6-8 hours before your screening, take your medication as prescribed by your healthcare provider with water while fasting.
  • If you are screening in the afternoon, eat a light breakfast and skip lunch.
  • After your biometric screening has been completed and submitted, you will receive an email with a link to check your results and view your rewards!
  • You can return to the rewards tab at any time to view your progress towards earning rewards.

Q: What if I previously had blood work done with my Primary Care Provider (PCP)?

If you had blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol, and triglyceride levels tested within 180 days before your Healthy Lives screening, bring them with you to your appointment.  We can use your results.

Q: What measurements does the screener take during a biometric screening?

A biometric screening includes height, weight, waist circumference measurements, Body Mass Index (BMI), blood pressure, heart rate, blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides, and a tobacco status test.

Q: How do I view my biometric screening results?

After completing your screening, you will have a one-on-one health coaching session to review your screening results.  

You may also view your screening in the inHealth app or web-based portal.  

  • Click on Screening Results in the left menu
  • Here you can see your current results side-by-side with your 1st screening results in the Member Results Dashboard
  • You may also pull up in-depth summaries by clicking “Download My Results”

Q: How often do I need to complete a Health Risk Questionnaire and perform a biometric screening?

Employees should complete the Health Risk Questionnaire and attend a biometric screening each program year.  The Health Risk Questionnaire is available in the inHealth app or web-based portal and must be completed no more than 90 days before your biometric screening.

Q: Can I complete my biometric screening with my primary care provider (PCP) through an annual wellness visit?

Yes.  An employee may complete their biometric screening with their primary care provider (PCP).  The employee must complete the Health Screening Questionnaire in the inHealth app; the primary care provider must record all biometric screening data on the Alternative Screening form at the time of their wellness visit.  The employee’s responsibility is to submit screening results to Healthy Lives and schedule a one-on-one coaching session to review screening results.

Q: Can I complete my annual wellness visit with my OB/GYN?

No.  Team members must complete an annual physical exam with a Primary Care Provider (PCP), which includes one of the following: Primary Care Physician, Nurse Practitioner, or Physician’s Assistant with the following practices: Family Practitioner, Internal Medicine specialist, Geriatricians, Pediatricians, or General Practitioners.

Q: What if I did not earn the wellness incentive for Body Mass Index (BMI), blood pressure, and/or blood glucose during my biometric screening?

Contact your Healthy Lives health coach to learn how to enroll in one of our programs to help you achieve your goals.

Q: If I improve my biometrics screening results during the program year, how do I submit my new values?

If you have improved your biometric screening values, you may submit a Wellness Attestation form with your new biometric data.  Wellness attestation must include all documentation and be submitted by October 31, 2021.

Q: What does Healthy Lives do with my screening information?

Healthy Lives uses all information collected from your health risk questionnaire and biometric screening results to provide you with information to understand your current health and potential risk and a customizable action plan.  Healthy Lives may offer you services such as one-on-one health coaching, wellbeing programs, and educational content based on your screening results.  You also are encouraged to share your results or concerns with your doctor.

Kinesics

In partnership with Healthy Lives, Kinesics is a platform to improve balance and mobility, a personalized plan to address your individual needs. During your Healthy Lives screening, you will receive a full range of motion evaluation and a one-on-one results review. Team members receive a customized flexibility and mobility program that is 100% unique to your body and includes video tutorials, so you feel confident about executing your program.

Q: What is a Kinesics Evaluation?

The Kinesics evaluation is a series of range of motion measurements used to identify joint and muscle restrictions that can cause pain or limitations with movement and overall function.

A professional will guide you through a series of simple movements and measure joint angles with a digital measurement device. It shouldn’t hurt, but if you feel any pain or discomfort with a movement, you can tell the professional, and he/she will not proceed with that measurement.

Q: How can I prepare for my evaluation?

  • We recommend not stretching or performing strenuous activities before arriving for your appointment. You can remain clothed in your work attire if it’s a shirt and pants – no dresses or skirts, please.
  • It’s best to wear non-restrictive clothing and remove any outerwear (blazers, jackets) before your evaluation to ensure you have a full range of motion to capture accurate measurements.
  • You will be asked to remove your shoes for a portion of the evaluation.

Q: How long does it take to complete the Kinesics evaluation?

  • The Kinesics assessment can take up to 15 minutes and includes a brief review of your results with your evaluator in the Kinesics app.
  • Your Kinesics assessment is part of your Healthy Lives screening appointment.

Q: How does the Kinesics evaluation affect my employment?

Your evaluation does not affect your current employment status. 

Q: What does Kinesics do with my information?

Your evaluation data is shared only with Healthy Lives.  Together, we can better understand how your body moves and provide you with the resources to help you feel better and move better at home and work.

Kinesics will work with you (and, if you wish, your doctor) to support and supplement an injury management care plan and decrease the duration of your medical intervention, getting you back to feeling and moving better!

Q: Is this a Pass or Fail exam?

No. There are no right or wrong human bodies!  The Kinesics program design is to identify areas of your body that are vulnerable to pain and injury before it becomes a problem.  If your results fall into a higher risk, a Kinesics professional will design a Custom Mobility Program for you to do for a few minutes each day, at work or home.  Team members should return in three (3) months for a follow-up evaluation to see how you are progressing and update your program.  The goal is to keep your body moving the way God intended!

Q: My results show I fall into the at-risk category; now what?

Upon completing your assessment, your Professional will briefly review your results in the Kinesics app. You will receive a results summary sheet that defines your ‘MHI’ percentage and the top five (5) restricted measurements from your evaluation.  If you fall into the at-risk category, your Professional will design a Custom Mobility Program for you based on the restricted measurements identified in your assessment. You should return in three (3) months for a follow-up evaluation to track your progress and update your program until you are no longer in the at-risk category.

Q: Does a severe measurement mean that I am going to be injured?

No. These measurements are not diagnostic or predictive of MSK pain or injury. A measurement outside of the reference range is not a certainty for injury, just as addressing measurements outside of the reference range is not guaranteed to prevent injury.  Poor posture, muscle imbalance and asymmetry, and joint restrictions are all risk factors of injury. Kinesics address these risk factors by ensuring the correct freedom of movement in all muscles and joints to reduce the risk of pain and injury from unnecessary wear and tear. A misaligned body is like a misaligned car. Wear and tear occur, increasing your risk for injury and hindering you from doing the things that bring you joy.

Q: How do I download the Kinesics app?

The Kinesics app is available for download in the Apple and Google Play App Store.

Step 1: Tap the App Store application on your device, then enter ‘Kinesics’ in the search box. Tap on the Get or Install button to download.

Step 2: Create a password.  Make sure you can access your email to open the email from Kinesics with your temporary password in it. (If you didn’t receive an email from Kinesics, check your spam or junk folder.)  Click on the link in your email to create a password.  Enter your new password into the data fields, then tap the set new password button to save.   The link to create a new password will expire within 24-hours after account creation.

Step 3: Login to the app.  Open the Kinesics app on your device, enter your username and password, and then Tap the Login button.

Q: How can I reset my password on the Kinesics app?

Go to www.kinesicshealth.com/password and enter your FMOLHS email address. Kinesics will send an email with a link to reset your password. Follow the directions in your email to reset your password.

Rewards

Each participating team member who meets the goals of his or her personal health and well-being plan is eligible to receive an annual cash incentive payment. The plan begins with biometric screening, and then progress is measured before the end of the benefit year. Results are measured for blood pressure, blood glucose, BMI, PCP visits, cancer screenings, and Advance Care Planning completion.  Team members who participate in a screening in 2021 are eligible to earn up to $550, paid in the first quarter of 2022.  Review the 2021 Annual Rewards below.

Q: Can you tell me about the wellness rewards?

You can earn a cash incentive of up to $550 each program year by working toward a healthier lifestyle. The Healthy Lives program year is from November 1, 2020, to October 31, 2021.  To earn a cash incentive, team members should:

  • Complete a Health Risk Questionnaire (HRA)
  • Schedule and complete a Healthy Lives screening before October 31, 2021
  • Complete an annual wellness visit with your primary care provider (PCP)
  • Achieve healthy ranges for Body Mass Index (BMI), blood pressure, blood glucose, and cholesterol levels. If you do not achieve one or more of these health goals during your biometric screening, you may complete one of our complementary programs, such as Go Healthy! Weight Management, Diabetes Prevention, or one-on-one health coaching.

Q: When will my wellness incentive be available to me?

The incentive payout will be the first quarter of 2022.

Q: If FMOLHS no longer employs me, can I still receive my wellness incentive?

No. Employees who have an active employment status on the incentive payout date are eligible to receive a payout. 

 

If it is unreasonably difficult, due to a medical condition or if you do not feel comfortable due to COVID-19, for you to achieve the standards for the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under the program, call Healthy Lives at 1-855-426-4325 and we will work with you to develop another way to qualify for the reward.
