McLeod Health is committed to improving the health and wellbeing of our members and their families. As an employee, you have access to the benefits of the McLeod Healthier You Program. Spouses on the McLeod health insurance plan are also eligible.

McLeod Healthier You offers FREE services! Which include:

➢ Annual Biometric Screening

➢ Personalized Care Management

➢ Diabetes Self-Management Education 

➢ Assistance with diabetic testing supplies and select diabetic oral medications

➢ 12-Week Tobacco Cessation Program

➢ Assistance with scheduling Preventative Screenings

➢ Employee Assistance Program referrals

By completing the annual biometric screening, members gain access to the inHealth App, which provides you with personalized health tracking and educational material!

In addition to the free programs offered, McLeod Healthier You now offers employees, spouses, and dependents 13 years of age or older access to healthcare in the form of sick visits, wellness exams, and primary care through the McLeod Healthier You Care Center.

McLeod Healthier You Care Center is available:

 Monday – Friday from 8:00 am to 5:00 pm.

 

 

Important Deadline Information:

  1. Insurance premium discounts will begin January 1, 2022 if HRA and Biometric Screening completed by November 19, 2021.
  2. If you miss the November 19, 2021 deadline, you can make an appointment to screen prior to April 15, 2022 and your insurance discount will begin in May 2022.

  1. Health Risk Assessment (HRA) and Biometric Screening
  • McLeod Health employees and spouses covered by McLeod Health insurance will each receive a $25 per month premium reduction ($50 total per month) in 2022.
  • If the spouse chooses NOT to complete the HRA and Biometric Screening, the McLeod Health employee will still receive the $25 per month premium if they have completed the HRA and Biometric Screening.
  • The HRA is recommended to be completed on your cell phone by downloading the inHealth App or visit https://portal.inhealth4change.com. You can use your personal or work email while creating an account. The company code is MCH.
  • Biometric Screenings will be held from October 1, 2021 until November 19, 2021.
  • The Biometric Screening Schedules will be posted on the McLeod Health Compass under the Living Well section, the Employee Health calendar, and Click here for Screening Flyer link
  • McLeod Healthier You will also accept results from your provider’s office, if labs were completed within 90 days prior to screening.
  • If you miss the November 19, 2021 deadline make an appointment to screen prior to April 15, 2022 and your premium reduction will begin in May 2022.
  1. Tobacco Use Screening

*New this year! All employees and spouses enrolled in the McLeod health insurance must prove their tobacco status to avoid the $50 monthly surcharge.

  • McLeod Health employees and spouses covered by McLeod Health insurance who
    • Fail to complete the tobacco use test as a new hire and annually;
    • Test positive for tobacco use or disclose tobacco use and fail to complete an approved Tobacco Cessation Program will each be subject to a $50 premium surcharge per month in 2022 ($1200 total per year).
  • If the spouse chooses not to complete the Tobacco Use Screening, test positive for tobacco use, and fails to complete an approved Tobacco Cessation Program, the McLeod Health employee will be subject to the $50 per month premium surcharge in 2022 ($600 total per year).
  • If you miss the November 19, 2021 deadline make an appointment to prove your status prior to April 15, 2022 and your tobacco surcharge will begin in May 2022.

New users: Download the inHealth Wellbeing app to register. If you don’t have access to a smartphone, you can conveniently login on a PC at https://portal.inhealth4change.com

  • Click on “Here” to create an account
  • Enter company ID: MCH
  • Press “create” to submit information

Complete Health Risk Assessment (HRA) Questionnaire & Schedule Your Appointment

  • Visit the mobile app or web portal at https://portal.inhealth4change.com/ and locate the screening tab to the left of your screen.
  • Once you’ve clicked on the screening tab, you’ll see the option to complete your Health Risk Assessment Questionnaire.
  • After completing and submitting your Health Risk Assessment Questionnaire, you will be directed to complete your biometric screening.
  • Click “Complete Biometric Screening” for instructions on how to complete your screening.

Attend Screening

  • Have employee ID number and/or spouse’s employee ID number available.
  • Fast 6 – 8 hours before screening. You may take any medications with water while fasting. If you are screening in the afternoon, eat a light breakfast and skip lunch.
  • If you had blood glucose and cholesterol levels tested within the last 90 days before your screening, bring your lab results with you.
  • After your biometric screening has been completed and submitted, you will receive an email with a link to check your results and view your rewards!

View Your Results and Rewards

  • Click on Screening Results in the left menu.
  • View your current results side-by-side with your first screening results in the Member Results Dashboard.
  • You may also pull up in-depth summaries by clicking “Download My Results.”
  • You can return to the rewards tab at any time to view your progress towards earning rewards.

Q: Why should I screen?

A: Screening opens the door for a free wellness screening, a comprehensive health risk assessment, one-on-one health coaching sessions, and a monthly insurance premium reduction. Healthier You biometric screenings are designed to identify certain health problems and provide a “snap shot” of your current health.

Q: How does the health risk assessment and biometric screening work?

A: Four easy steps:

  • From our mobile app or website, go to the Screening tab and complete the “My Health Risk Questionnaire”
  • Book your appointment for your biometric screening
  • Complete the screening, which includes measurements of your height, weight, waist, blood pressure, and blood sample for glucose and cholesterol levels
  • Review your results in the mobile app to better understand YOUR health OR call to schedule an appointment with a Healthier You care manager to review your results

Q: How do I get started?

A: To begin:

  • Download the inHealth app on your smartphone. If you do not have a smartphone, visit inhealth4change.com to create an account and complete your Health Risk Assessment (HRA)
  • Enter Company ID: MCH
  • Click “Create Account”

Q: What do I do if I forgot my email address to access my inHealth mobile app account?

Q: What do I do if I forgot my password to the inHealth mobile app?

A: Click “Forgot Password” on the sign in screen. A temporary password will be emailed to you.

Q:  If I test positive for tobacco during my biometric screening or self-attest that I use tobacco on my Health Risk Questionnaire, what steps do I need to take to avoid the tobacco surcharge?

A:  You will need to complete a complimentary tobacco cessation program. The program must be completed by December 15th to earn the surcharge January 1, 2022 or by April 15th2022 to earn the surcharge beginning May 1, 2022.
