Our systems were originally developed in 2009 as our founding organization began to prepare for health care reform, with an emphasis on developing people, processes, and technology to support a transition from volume to value-based care. Employers in our communities urged us to help find solutions for the unsustainable increases in health care costs that were impacting their bottom lines. As a large, self-insured employer, we realized we were experiencing similar trends, and Healthy Lives was born.
Built on a myriad of best practices from many industries, we created a clinically-oriented, data-driven model for population health true to our provider origins — focusing both on the management of high risk individuals and the prevention of future risk via holistic lifestyle and behavioral interventions. The program yielded impressive results for our founding organization, resulting in cost savings, flat premiums for 5 out of 6 years, improved health outcomes, and a two-thirds reduction in the high risk segment of the population.
We’ve learned this is about getting back to the basics of good primary care and improving outcomes for the communities we serve.
