When it comes to our health, there are aspects within our control and outside of our control. Our goal is to help manage those aspects within our control through delivering valid information, offering support and helping provide the extra accountability that so many of us need. We believe a healthy lifestyle does not include absolute thinking; rather, it is about finding a balance that works for each individual.
We encourage those we coach to focus on eating whole foods and less prepackaged foods, but sometimes you have to give yourself the freedom to enjoy the foods you love and not feel guilty about it. To achieve and maintain a healthy lifestyle, it is not necessary to train for a body building competition or marathon, but it is important to move more and sit less.
Our ultimate goal is to help those we coach experience the benefits of a healthy lifestyle such as increased energy levels and simply feeling better. Numbers on the scale tell a small part of the story, but the scale cannot measure feelings and emotions. Our focus is on you and your journey, because the work to live a healthier life does not end, it changes, just as people do.

Tobacco/nicotine use is the leading cause of preventable death in our nation today. Because of this, we actively support a healthier, tobacco-free workforce that models a healthy lifestyle for our patients and community.
As a participant of wellpower, you have the opportunity to earn a tobacco/nicotine surcharge waiver for working toward a healthier lifestyle.
Participants will earn the surcharge waivers throughout the year by making healthy lifestyle choices such as:
Q: Can you tell me about the tobacco/nicotine surcharge waiver?
A: All team members and spouses enrolled on/enrolling in the health plan for 2024, who complete the HRA and screening by November 21, 2023, are each eligible to earn $1,300 for an individual and $2,600 ($50 per bi-weekly pay period per individual) for a couple in annual tobacco/nicotine use surcharge waivers.
* Individuals who test positive for tobacco/nicotine use or attest to using tobacco/nicotine during a wellpower screening can earn the surcharge waiver by completing an approved tobacco cessation program. Employees and spouses on the health plan will have the opportunity to utilize a designated tobacco cessation program provider. Upon successful completion of the tobacco cessation program before April 1, 2024 (or within 90 days of the effective date for joining the plan), the surcharge will end and the participant will be refunded the surcharge paid.
Q: Can I just complete the tobacco/nicotine test to avoid the surcharge?
A: No. Team members and spouses enrolled in the health plan must complete a wellpower screening, including the lab draw to be eligible to have the tobacco/nicotine surcharge waived in 2024. Without a screening you will not be eligible to have the $50 (per person per bi-weekly pay period) tobacco/nicotine surcharge removed through December 2024.
Q: I am a new team member or newly eligible to enroll in health benefits. How long do I have to complete a wellpower Biometric screening?
A: New health plan enrollees who enroll during the year must complete a wellpower Health Risk Assessment and Biometric Screening (including the tobacco/nicotine screening) within 30 days of coverage effective date on the health plan to qualify for surcharge waivers. Spouses enrolling on the plan must also complete a wellpower Health Risk Assessment and Biometric Screening (including the tobacco/nicotine screening) within 30 days of coverage effective date on the health plan to qualify for surcharge waivers. If you are enrolling for coverage beginning January 1, 2024 during Open Enrollment you are required to screen before November 21, 2023.
A biometric screening is a series of health tests designed to offer insight into your overall health. A wellpower biometric screening includes measurements of height, weight, waist circumference, blood pressure, heart rate, blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides and a nicotine/tobacco test.
Q: Who is eligible to complete a biometric screening?
A: Team members enrolled on/enrolling in the Stormont Vail health insurance plan (medical coverage) and spouses enrolled on/enrolling in the Stormont Vail health insurance plan (medical coverage) are eligible to complete a biometric screening.
Q: Why should I complete a biometric screening?
A: There are so many reasons!
Q: How do I complete a biometric screening?
A. Visit the mobile app or web portal at https://portal.inhealth4change.com/ and locate the screening tab to the left of your screen.
Once you’ve clicked on the screening tab, you’ll see the option to complete your Health Risk Assessment Questionnaire. Be sure that you enter your initials and click submit at the end of the questionnaire to complete the consent process. Once your questionnaire is complete, you will see a green check mark indicating complete along with date completed.
After completing and submitting your Health Risk Assessment Questionnaire, you will be directed back to the screening page to schedule your biometric screening.
Schedule an appointment for your wellpower biometric screening. To ensure efficiency and quality, no walk-in appointments will be allowed. All screenings will be by APPOINTMENT ONLY. Book early to get your preferred date, time and location.
Attend your wellpower screening to complete your lab and biometrics measurements including blood glucose, cholesterol (total cholesterol, LDL, HDL and triglycerides), tobacco/nicotine screening, height, weight, waist circumference, blood pressure, and heart rate.
After your biometric screening has been completed and submitted, you will receive an email with a link to check your results!
Q: What do I do if I forgot my password to the inHealth mobile app?
A: Click “Forgot Password” on the sign-in screen. A temporary password will be emailed to you.
Q: Should I fast for my screening?
A: Fasting 8 – 12 hours before your screening is recommended for best results, but not required. You can take any medications with water while fasting.
Q: What if I just had blood work done with my medical provider?
A: If you had blood glucose and cholesterol levels tested within 90 days of your wellpower screening date, bring your lab results with you to your screening and we will use those results. You must still complete measurements for height, weight, waist circumference, blood pressure, heart rate and the tobacco/nicotine screening.
Q: What does wellpower do with my screening information?
A: The information from your HRA and the results from your biometric screening will be used to provide information to help understand your current health and potential risks and may also be used to offer you services through the wellpower program, such as well-being programming and content.
Q: What if I am unable to participate due to a limitation or disability?
A: If you are unable to take part in any of the health-related activities or meet any of the health outcomes required, you may be entitled to reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by completing and submitting the form found at https://svnet.stormontvail.org/HR/pay-benefits/Pages/wellpower.aspx.
Q: What measurements must be taken during a biometric screening?
A: Height, weight, waist circumference, blood pressure, heart rate, blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides and your tobacco status.
Q: How do I view my biometric screening results?
A: Login to your inHealth Wellbeing account, click on the Screening tab to view your results.

Q: How often do I need to complete the Health Risk Questionnaire and perform a biometric screening?
A: Annually for employees and spouses on the medical plan during the fall screening blitz.
New team members/enrollees during the year are required to complete a screening within 30 days of coverage effective date and will also be required to complete a screening during the annual fall blitz to be eligible to receive tobacco/nicotine surcharge waivers.
Q: Can I complete my biometric screening with my primary care provider (PCP) through an annual wellness visit?
A: No.
Q: What if I complete my screening but do not complete the lab draw?
A: The lab draw (or proof of labs within 90 days prior to your screening date) is required in order to complete the annual screening and for participation in wellpower.
Q. What is the Stormont Vail wellpower Diabetes Management Program?
A. The Stormont Vail wellpower Diabetes Management Program will help you take an active role in managing your diabetes by working with our Health Coaches and Care Managers on your personalized care plan. Our approach focuses on improving your overall health and wellbeing while helping you learn how to manage your diabetes better. Stormont Vail wellpower Health Coaches and Care Managers, in partnership with your healthcare provider, will aid you in setting and achieving goals. Participants meeting all requirements of the program receive 100% coverage of approved medications and supplies, including medically necessary needles, syringes, lancets, test strips, glucometers, insulin pumps, and insulin pump supplies (please see Stormont Vail Health formulary).
Q. Who is eligible to participate in the Stormont Vail wellpower Diabetes Management Program?
A. Team members, spouses, and dependents enrolled in the Stormont Vail Health Plan who have been diagnosed with type 1 diabetes, type 2 diabetes, gestational diabetes, insulin resistance or prediabetes are eligible.
Q. What is the benefit of participating in the Stormont Vail wellpower Diabetes Management Program?
A. Participants enrolled in the Stormont Vail wellpower Diabetes Management Program will receive 100% coverage of approved diabetic medications and supplies, including medically necessary needles, syringes, lancets, test strips, glucometers, insulin pumps, and insulin pump supplies (please see Stormont Vail Health formulary) when filled at Stormont Vail Retail Pharmacy or Stormont Vail Mail Order Pharmacy. Durable medical equipment (DME) such as pumps and pump supplies not available at the Retail Pharmacy can be purchased directly from DME provider (Please see Stormont Vail Health formulary). Additional steps required for the purchase of DME directly from provider. Please reach out to wellpower@stormontvail.org for questions.
Health Coaching: enrolled participants will be assigned a Health Coach or Care Manager to assist with improved management of their disease and overall wellbeing. Health Coaches or Care Managers will work with enrolled participants by phone, email, or in-person to establish personalized diabetes and health management goals and monitor results.
Q. How do I enroll in the Stormont Vail wellpower Diabetes Management Program?
A. Complete a Stormont Vail wellpower Screening.
Upon receipt of a completed Enrollment Form, a Stormont Vail wellpower representative will contact you to complete the enrollment process.
Completed forms should be sent to Stormont Vail wellpower via email, wellpower@stormontvail.org or faxed to (785) 354-5442. The provider verification of diagnosis form must be faxed by your provider. Participants enrolled on or before the 25th of each month will see cost savings the first of the next month.
Q. I have not completed a Stormont Vail wellpower HRA and Biometric Screening. Can I still participate in the Stormont Vail wellpower Diabetes Management Program?
A. The wellpower HRA and Biometric Screening must be completed during the current Stormont Vail wellpower program year to be eligible for the program.
Q. What are the requirements to remain enrolled in the Stormont Vail wellpower Diabetes Management Program and maintain the pharmacy benefit?
A. In order to remain enrolled in the program and maintain the pharmacy benefit participants must meet the following minimum requirements.
Q. How do I access the Stormont Vail wellpower Diabetes Management Program pharmacy benefit?
A. Once enrolled in the program participants must present a prescription to purchase diabetes medications and supplies at the Retail Pharmacy or SVH mail order. (DME such as pumps and pump supplies not available at the Retail Pharmacy can be purchased directly from DME provider. Additional steps required for the purchase of DME directly from provider. Q. What medications and supplies are covered at no cost to enrolled participants?
A. Provider prescribed medications and supplies, including medically necessary needles, syringes, lancets, test strips, glucometers, insulin pumps, and insulin pump supplies to treat diabetes, that are included on the SVH Employee Health Plan Formulary will be covered at no cost to enrolled participants.
DME such as pumps and pump supplies not available at the Retail Pharmacy can be purchased directly from DME provider. Additional steps required for the purchase of DME directly from provider. Please reach out to wellpower@stormontvail.org for questions.
Members must present a prescription to purchase diabetic medications and supplies using the pharmacy benefit plan to receive these medications and supplies at no cost.
Q. What happens if I am not compliant with the requirements of the program?
A. Enrollment in the Stormont Vail Health wellpower Diabetes Management Program benefit will cease on the date the Health Coach or Care Manager determines that the participant is no longer compliant with the requirements of the program. At that time, all prescriptions for diabetes supplies and medications will be subject to standard copays and deductibles of the pharmacy plan elected during health plan enrollment.
Disenrolled participants continue to be eligible to participate in Stormont Vail wellpower health coaching, care management and wellbeing programming.
Q. Can I re-enroll in the program once I have been disenrolled?
A. Participants who have been disenrolled from the Stormont Vail Health wellpower Diabetes Management Program are not eligible for the Stormont Vail Health wellpower Diabetes Management Program pharmacy benefit until the start of the next benefit plan year.
