We invite all employees of participating companies to participate and earn wellbeing rewards. Complete your Health Risk Assessment Questionnaire (HRAQ) and biometric screening during your annual wellness visit to earn up to $350. Plus, engage in various wellness activities to boost your total rewards to $500.
You can earn rewards from July 1, 2024- June 30, 2025. Rewards will be paid out quarterly as part of your regular paycheck. Payouts will occur in January, April, and July.
Please note that the maximum annual reward is capped at $500.
How do I get started?
Use “II” as the Company ID when creating your account.
Our Wellbeing Program includes various components to address your diverse wellbeing needs. We offer a range of resources and activities designed to improve overall wellbeing and promote healthy behavior change.
Health Risk Assessment ($100): Complete a confidential Health Risk Assessment (HRA) Questionnaire in the inHealth mobile app or portal. Once you have submitted your Biometric Screening Form, you will be able to access a report in the inHealth app and portal on the Screening page with valuable information about your risk for developing chronic disease and ways you can prevent it.
Complete Annual Wellness Visit ($150) – Participate in an annual wellness checkup with your Primary Care Provider (PCP). This essential visit plays a critical role in preventive care, early disease detection, and effective health management – thereby reducing overall healthcare risks. Have your primary care physician fill out the Provider Form located on the Screening page in the inHealth app or portal. Email the completed Provider Form or a printout of your results from MyChart to service@inhealth4change.com by 6/30/25. *Provider forms with service date will be accepted after 6/30/2024.
Complete a Biometric Screening ($50) – Biometric screenings serve as an essential diagnostic tool for identifying potential health risks, thereby enabling timely interventions and promoting overall wellbeing. A biometric screening can be performed during your Annual Wellness Visit. Ask your provider to complete and email the completed Provider Form or a printout of your results from MyChart to service@inhealth4change.com by 6/30/25. *Biometric screening results must be collected after 6/30/2024.
Self-Management Educational Programs (Earn $25 per program) – Complete a variety of self-management programs under Nutrition, Physical Activity, Sleep, or Career & Financial sections. Each self-management program is worth $25 points.
You can earn $300 for completing the Healthy Minds program in the Emotional & Intellectual section.

Complete the attestation in the Health Tracking section of the mobile app or online portal once you have completed the program.

Emotional & Intellectual Program ($300)
Healthy Minds – The Healthy Minds program is designed to support overall emotional wellbeing. This program is an online educational course consisting of 8 modules designed to help explore the foundational supports for positive emotional health. The program also includes tools and resources to help participants successfully handle life’s stresses and adapt to change and difficult times. Last day to enroll is May 1, 2025.
Physical Activity ($100): Complete an average of 90 minutes (about 1 and a half hours) of physical activity per week by tracking on the Health Tracking section or syncing your fitness device.

All benefits-eligible employees of participating companies are eligible to participate in the wellbeing incentive program.
How do I earn rewards?
Complete your Health Risk Assessment Questionnaire (HRAQ) and biometric screening during your annual wellness visit to earn up to $350. Plus, engage in various wellness activities to boost your total rewards to $500.
Progress towards earned rewards can be viewed in the Rewards tab. A green checkmark indicates earned rewards and will be paid out the following quarter.
