The Go Healthy+ program is an advanced program offered to Go Healthy! graduates to build on their prior success in the program. Healthy living and weight management is more than diet and exercise. It requires habit and lifestyle changes. Change can be difficult and Go Healthy+ is designed to support you on the journey to healthy living. Participants must have graduated from Go Healthy! within the last 12 months.
Save money while we help you manage your diabetes! The Diabetes Management Program offers the benefit of waived copays and deductibles for prescribed diabetes medications and supplies by meeting program requirements such as completing the biometric screening and engaging in health coaching or care management.
The Diabetes Prevention Program (DPP) can provide you with specialized health coaching, valuable resources and tools to help you lower your risk of developing Type 2 diabetes. Who knew that a diabetes prevention program could be fun, engaging and taught in an environment where you can learn, laugh, share stories and develop new habits? This program will help you manage nutrition, increase physical activity, implement prevention strategies and monitoring techniques.
As a member, you have access to a FREE 12-week program that pairs you with a certified health coach who will be with you each step of the way to help you quit successfully. Upon enrollment, your health coach will work with you for 10-15 minutes weekly to create a customized plan and identify your triggers. Studies have shown that tobacco cessation programs paired with health coaching are proven to be more effective to help users change behavior and quit for good.

Work with a wellbeing specialist to set personal goals that meet the demands of your lifestyle.
